Use this inspector's checklist guide to help with on-the-spot recording of findings and comments. This hecklist helps to clarify inspection responsibilities, controls inspection activities and provides a report of inspection activities.

Before use, the employer must ensure that:
The operator is competent*
The forklift platform is equipped with guardrails, travel restraint or fall-arresting system and attached to an anchor point provided by the manufacturer or approved by a professional engineer.
If a fall-arresting system is used, that it does not interfere with raising and lowering the platform.
A copy of the manufacturer’s operating manual for each forklift used on the premises is available to all operators.
The platform is designed and constructed of sufficiently strong materials or, if a manufactured platform used, that it is maintained and dismantled according to the manufacturer’s specifications.
Before use, the operator must ensure:
The forks are fitted properly into the forklift guides on the platform.
The platform is secured to the forklift carriage in a way that prevents inadvertent movement.
The work platform is inspected before each use, as well as during the monthly inspection, according to manufacturer specifications, and can include:
Tie locks
Anchor point
Snap hook attached to the free end of the nylon safety strap
Gate latch(es)
Handrails
Lanyard attachment points
Casters (if equipped)
Product labels
Welds and damage
During use of the forklift with a platform, ensure:
The forklift is on a stable and level surface when a worker is on a platform mounted on the forklift.
When a worker is on a work platform mounted on a forklift, the operator remains at the controls when the platform and the forklift are in the elevated position.
The work platform is only used for lifting people and necessary tools and equipment.
To immobilize the forklift vehicle against unintended movement before and during the time that workers are on the platform.
The operator is in control of the forklift while workers are on the platform.
