This procedure is a form that needs to be filled out regarding property damages that occur at our sites. Please fill out as many details as possible so we can use for insurance purposes or financial reimbursement. We will use this report to attach to all invoices that accrue during repairs.

This form must be completed within 2 hours of the YWCA employee learning of the incident. Please check the box below in which the incident identifies under.
