What equipment will be used for elevated work?
Ladder Condition
Footing
Rungs are not bent, broken or missing
Rails are not damaged
Ladder extensions properly lock in place
Scissor Lift or Snorkel Lift
Daily inspection has been performed
Cage has a locking gate
Cage floor is clean of any oils and trip hazards
Cage slide out moves with ease and has locking pins
Forklift w/ Man Basket
Forklift daily inspection has been performed
Cage has securement mechanism to secure to the mast
Cage has locking gate
Cage floor is free of any oils and trip hazards
Cage railings are not damaged
Harness Condition
Harness straps are not frayed, cut or torn
Buckles securely latch
Life Line is not torn, frayed, damaged, or stretched
Manufacturer labels are present and ledgable
