Center
Electrical
Lighting: All fixtures, bulbs and lenses are in place and functional.
Outlets, light switches, wall box covers, data and phone covers: All covers are in place and not cracked
Electrical outlets: All outlets have the approved child safety plugs inserted in them and in good condition.
Electrical cords, extension, data and phone: No piggybacking. All of them are placed along a wall and are not in the way of tripping or where children can gain access to them.
Appliances: Large and small appliances are in good working condition.
Exhaust Fans: All exhaust fans (i.e. restrooms and kitchens) are functional and in good working order
Walls and Coverings
Walls: All Interior walls are in good condition and need no repair. No holes, large nicks, and chipped paint. Make sure all trim and ADA hand rails are in place and in good condition
Baseboards: Secure, and in good condition
Plumbing
Faucets and sinks: All are in good working order. Check both hot and cold side to make sure both are functioning. Look in if equipped with a vanity for any leaks and that they are draining good
Paper towel, soap, toilet paper and hand sanitizer dispensers: Check to make sure they are all in good working order and functioning as intended
Sink and floor drains: Ensure that there is no concerns with odor
Kitchen grease traps: Make sure they are no over flowing, check date on last service
Water and Sewer: Check water wells, lift stations, drain fields, water mains, pipes and water meters for any leaks or needed repairs
Toilets and Urinals: Check to make sure they are flushing, check hard ware ( tank, tank parts, handles , bowl, seat, mounting bolts, supply lines, valves) are in good working order
Windows and Doors
Windows and coverings: Check the following items (glass, window sill, frame, locks, and window treatments
Doors and hardware: Check the (door condition, locks, threshold, frame, weather stripping, site glass,) Also check the door closure to make sure the door is closing on its own as needed.
Door Alarms / Intrusion contacts: Check to make sure they are in good working order and secured to the wall.
Emergency Egress and Fire drills
Emergency egress / exit lights: Check all illuminated and non exit signs are in place and in good working order. Test illuminated exit lights and make sure they are in good working order.
Emergency lighting: Check all lights to ensure they are in good condition and are fully functional.
Fire Extinguishers: Check to make sure the extinguisher certification is updated and has not been tampered with. Check the pin to make sure it is still in place, check house to make sure there is no cracking or cuts in it, make sure the extinguisher is full.
Fire Notifying Devices: Provide the date of the last inspection completed for the following items: Fire Sprinkler and suppression sysytem if applicable and Fire Alarm system.
Evacuation routes, fire drill and disaster preparedness plans: Check these items are accessible, up to date and current
Interior
Hallways / Corridors: Make sure all hallways and paths for emergency egress are free and clear of clutter and furniture
Ceilings: If equipped with ceiling grid / tiles make sure all tiles are in place, no holes, stains etc. Drywall or popcorn in good condition.
Flooring: Check VCT, carpeting, tile and baseboards are in good condition and not torn, ripped missing and in need of paint
Carpet: Clean not frayed and in good condition
Storage Cabinets and Closets: Check that the storage cabinets/closets are kept closed and locked when not in use.
Air Conditioning: Supply and return grills are clean / checklog or filters to confirm when the last time filters were changed
Thermostat: Check and make sure everything is working fine with the thermostats
Room Temperature , Air Flow, and Odors: Check room temps to see if they are at comfortable levels, take note of the air flows for the rooms, and check for any kinds of odors
Cleanliness: Check the condition of the floors, check to see if the are clean and free of scuffs, marks and to see if they are being maintained.
Bathroom: Restrooms supplies are stocked, fixtures, floor and mirrors are clean. No odor present.
Cleanliness: Check trash cans, sinks, fixtures blinds hard services partitions and toilets for normal upkeep.
Cleanliness: Daily custodial check list log completed and up to date
Pest Control: Any evidence of pests. Check the log to confirm when the last time pest control was performed. Take note of any critters
Building Exterior
Stairs and Steps: Check all steps for any needed repairs
Ramps: Check any and all ADA ramps for any deformities and report accordingly
Walls, siding and skirting: Exterior walls and siding is in good repair. Skirting is in good condition and secure on modular buildings.
Building Exterior: Check surrounding concrete sidewalks, ramps, curbs etc. for any breaking, splitting, or lifting that would create a tripping and or fall issue
Storm Water Run Off: Check gutters, scuppers, drains, parking lot drains for anything that may be blocking the flow of water
Roof: Check the roof for any missing shingles. Also check the condition of the flashing,eaves,soffits, etc.
Electrical and Lighting: Check the condition of outdoor electrical outlets. Check the electrical meter and main shut offs. Check building and parking lot lights.
HVAC: HVAC equipment in good working order(i.e. units and controls)
Parking Lot: Check parking lot for any pot holes, any evidence of water pounding, check parking stops for any signs of damage
Grounds: Check fencing and gates for the following: latches are in place and functional, rails and mesh, as well as any missing hardware and that they are functioning correctly
Landscape: Check the following items landscape boarders, flower beds are free from weeds and mulch is present. The grounds are being maintained such as mowing, edging, weed wacked and blown off. Also check to make sure grounds are free of trash.
Playground
Equipment: All equipment is free from any broken parts, sharp edges, rust etc. Equipment is maintained no chipped paint
Surfacing: Surfaces have the appropriate level of loose materials and is at the certified depths
Playground: Play area is being maintained blown off, free from litter and debris including leaves. Excess mulch is on hard surfaces and toys are removed regularly
Shade Structure: Check for uneven surfaces, tree roots, holes in or low spots in green spaces, cracked and broken concrete
Trees and Vegetation: Green space is free from vegetation and foreign objects. Low hanging tree limbs are not present.

Pest Control: Check for ants, wasps, snakes, rodents etc. and report to pest control
Ponding: Check for any evidence of standing or ponding water
Sand and Water Table: Make certain that sand and water tables have the appropriate covers
Water Accessibility: Check the water fountains, hose bibs, shut off valves, for any leaks or signs of non functional items
Swings: Check S hooks on swing seats and any other items that they are being used to connect two items together
Equipment: Check all equipment to ensure it is properly anchored
Head Entrapment: Be sure any openings in equipment are less than 3 inches or more than 9 inches wide.
Playground: Check to see if any of the playground equipment or anything else is blocking the view of all equipment.
