Use this checklist to complete prior tasks before using the equipment. The checklist will help you prevent hazards that are usually unidentified because of the lack of pre-start inspections.

Date (Week commencing Mon.).
Day of the Week
TYRES
look for visual wear or damage. Are they in good condition?
FLUIDS
check oil, hydraulics, battery, fuel and coolant.
SEATING
check condition and adjustment
WARNING DEVICES
check visual and audible devices (strobe, horn, reversing beeper)
MAST
check for any wear to lift chains and guides, inspect hydraulic cylinders, look for any leaks
TINES
check for any signs of damage
CAPACITY
- check load capacity plate is fitted, legible and appropriate for the forklift/attachments
Start forklift and check:
CONTROLS
check all pedals and controls
BRAKES
check brake and parking brake
If any items above are deemed unsatisfactory, the forklift is to be tagged out and reported to the supervisor.
